Following Anesthesia Billing Guidelines Helps Expedite Claim Payments – Anthem BCBS
Anthem strives hard to process and pay all claims in a timely manner. Therefore, we want to provide you with some important billing guidelines you should follow that can help us expedite the payment of your claims for anesthesia.
Paper Submissions
Providers who submit paper claims should:
• Use the appropriate ASA CPT anesthesia code (00100-01999) with the appropriate modifier.
• Indicate the actual time of the service rendered, in Minutes, in field 24G of the CMS-1500 form.
• DO NOT report the base units on claims.
Electronic Submissions
Providers who submit claims via Electronic Data Interchange (EDI) should:
• Use the appropriate ASA CPT anesthesia code (00100-01999) with the appropriate modifier.
• Indicate the actual time of the service rendered, in Minutes, in the 465A record segment using an MJ qualifier. (MJ qualifier within the electronic claim record = minutes billed.)
